Pay minimum payment of $685.00 via Zelle (Required to confirm and save spot for the class)

$185.00     Registration Fee (non-refundable) 

$500.00     Tuition Down Payment 

$685.00     Minimum due to start registration process

$475.00    Balance remaining (pay at the end of theory class or before theory end)

Upload file/documents to registration section form (mandatory to start enrollment process & save spot)

  1. Government Photo ID Card (valid unexpired)

  2. Social Security Card (non-laminated)

  3. CNA Initial Application CDPH 283 B Form

  4. Live Scan Fingerprint Form   Live Scan Form  (Bring this form to live scan vendor) Sample BCIA Form


 Step 4: Congratulations!! School admission counselor will review your online registration application & document submission. You will receive a text or call confirmation regarding your registration. You will receive a welcome packet in the mail that include your school materials, or you can pick it up at the school. 

 Step 5: Please submit your Physical Exam and TB form Physical & TB Exam Form. (deadline before clinical start)
